Procedure to chnage name in DOB certificate for my 8 month old son

even i am looking forward to add krishnan to my son's name in birth certificate.

the process i heard is:

1. approach corporation office (in city limits). else i think municipality/village office.

2. they will issue a form. you should fill it and give them(not sure of the other process here).

3. then name change details should come in gazette

4. preserve this gazette one and then i think it will go thro the corporation office.

soon am goin to approach the village office since we reside in tambaram
